RE:

Approve UT1510 Change Order to Purchase Order #013418 for Emergency Waterline Repairs at 23rd and Massachusetts

Background

On August 24, 2015, staff was notified of a water leak at 23rd and Massachusetts Streets requiring immediate attention and repair, including replacement of 12 feet of waterline as well as asphalt pavement removal and approximately 120 feet of asphalt pavement replacement for three lanes of 23rd Street.

An original emergency purchase order was issued to R.D. Johnson for $14,000 prior to knowing the full extent of the repairs that would be needed at the site.

Project Status

The contractor’s site activities are now complete, with services totaling $42,287.72. These services include extensive traffic control in managing high traffic proximate to the work site. In accordance with the City’s Purchasing Procedures, we request Commission approval of the change order needed to increase emergency Purchase Order #013418 by $28,287.70 for a new Purchase Order total of $42,287.72.

Project Funding

Purchase is funded from the Utilities Non-Bonded Cash Construction.
