Please include the following item on the City Commission consent agenda for consideration at the December 18, 2012 meeting:

Project Description/History.  The 2012 Concrete Rehabilitation Program was awarded to RD Johnson Excavating Co., Inc. on April 17, 2012.  This concrete rehabilitation project includes intermittent curb repair and replacement, some adjacent concrete sidewalk impacted by curb replacement including access ramps, and various concrete street pavement removal and replacement.  The street curb and gutter and concrete pavement sections to be worked on are in poor condition with extensive concrete deterioration (see attached maps).

Project Funding.  The current contract amount is $490,656.00.  This project has had an overall increase in quantities and also projected quantity increases to continue completion of planned 2012 Concrete Rehabilitation street locations.  It is projected that by the completion of the project that there will be an overall increase of $200,000.00.  The funding to be utilized is from under run quantities from other completed 2012 contracted maintenance projects.

The contract amount will total $690,656.00.  The change order amount impacts the total contract amount but does not result in exceeding previously approved budget allocations.

Action Request.  Approve Change Order No. 3 to RD Johnson Excavating Co., Inc. in the amount of $200,000.00.
